Student Needs and Experiences Questionnaires

On Tuesday, October 27, two student questionnaires were sent to UW-Madison students – one to all undergraduate students, the other to all graduate students. The questionnaires ask students to share their needs and experiences during the fall 2020 semester. Both seek to help campus leadership better understand what has worked well and what has been challenging, and in turn help the university maintain its commitment to providing the support students need to be safe, healthy and successful.

Each questionnaire takes about 10-15 minutes to complete, and can be accessed with the unique link provided to each student via email. Participation is optional, but strongly encouraged. Students may choose to skip any question they do not wish to answer.

Undergraduate Student Questionnaire – Deadline: Monday, Nov. 9

Students will have the option at the end of the questionnaire to request that a campus representative contact them to help connect with campus resources. If students choose this option, they will be asked to share their contact information in this section of the questionnaire only. The rest of their responses will be kept confidential.

Graduate Student Questionnaire – Deadline: Monday, Nov. 16

Responses are confidential and will not be shared with advisors, committee members, program faculty, or others in a way that identifies the students.
